To obtain an ESMS account please fill out both the ESMS Access Request (obtained from the download button above) and the Azure App Registration (link below) . For the ESMS Access Request please fill out completely and sign the form using your CAC (Common Access Card).
For the Azure App Registration please fill out the top half only and do not sign this form as the signature is for the CERL ESMS POC signature. To get to the Azure Application follow the link below:
